Hope was witness to the Mercedes SUV in front of him being hit by a Metro North train at the crossing in Valhalla in which the driver, Ellen Brody, 49, of Edgemont, was instantly killed.

The train crash also claimed the lives of five train passengers and 15 were injured.

Hope occasionally asks himself if there was anything he could have done to prevent the tragedy, says the report.
